If you’re looking to improve your strength, mobility, and overall health, Talk Health Therapy team of qualified Exercise Physiologists help you move better and live easier by creating tailored at-home exercise programs.
We’re committed to helping you reduce pain and live more independently wherever you’re located across Brisbane, whether in the comfort of your home or at a local pool or fitness centre of your choice.
Our team covers the following five Aged Care Planning Regions (ACPRs):
Our mobile Exercise Physiologists focus on improving your overall health, mobility, and independence through targeted, evidence-based exercise programs. Our programs are designed to help you build your strength and flexibility, improve your balance, reduce the risk of falls, manage and maintain chronic conditions, and increase your overall fitness and confidence in moving.
We focus on providing you with genuinely sustainable, long-term health results that will help support your journey to better health and a more active lifestyle.
Our Exercise Physiologists provide you with personally designed programs to help you build strength, improve endurance, and enhance overall fitness – all done in the comfort of your home or at a location of your choosing.
We focus on exercises that help you improve your balance, coordination, and stability, which will help reduce the risk of falls so that you’ll feel more confident on your feet.
If you’re living with arthritis, diabetes, heart disease, or another chronic health condition that affects your daily living, we provide you with safe and effective exercise plans that help manage your symptoms and improve your quality of life.
Our mobile team supports you with targeted rehabilitation exercises that help if you’re recovering from a surgery, illness, or serious injury, helping you restore function and mobility at your own pace.
We start with a detailed assessment to understand your own unique needs, goals, and current physical abilities. This includes evaluating your strength, mobility, balance, and any existing health conditions. We also take the time to learn about your personal priorities and what activities or tasks you’d like to improve, making sure that our services align with your lifestyle and overall health goals.
After the assessment, we’ll collaborate with you and your carers to develop a tailored exercise program which will help build strength, improve your balance, recover from an injury, or manage a chronic condition. Your personal plan will focus on achievable, evidence-based strategies to support your physical health and independence.
With your overall care plan in place, our team then is ready to provide regular therapy sessions and services in the comfort of your home. We provide ongoing guidance, monitor your progress, and adjust the plan as required to keep you on track and achieve your health goals.
You don’t always need a referral to access our Exercise Physiology services. However, if you’re accessing services through the NDIS, a Home Care Package, or another kind of funding, you may require approval from your plan manager or provider. Private clients can self-refer directly to our team.
If you’re unsure, give us a call at (07) 2111 6983 or send an email to info@talkhealththerapy.com.au, and we’ll help you navigate the process.
Once we’ve received your referral, one of our friendly team members will contact you to confirm your details and discuss your goals. During the first appointment, we’ll conduct a comprehensive at-home assessment to assess your current strength, balance, mobility, and physical limitations in order to identify areas for improvement.
From there, we’ll work with you to create a personalised care plan that’s designed to enhance your independence and quality of life.
