Our CHSP exercise physiology services support older Australians to improve mobility, manage chronic conditions, and achieve better health and wellbeing through safe, tailored exercise programs designed to help you stay active and stay independent.
We start with a detailed assessment to understand your unique needs and goals, and how these fall under the CHSP. This includes evaluating your mobility, everyday activities and overall health, liaising with the aged care assessment team where needed, and learning what matters most to you.
After the assessment, we collaborate with you and your carers to set achievable goals that support independent living. We create a tailored care plan addressing your specific circumstances, whether that's improving mobility, reducing falls risk, or supporting your nutrition or communication.
With your care plan in place, our team provides regular therapy sessions in the comfort of your own home. We offer ongoing support, monitor your progress, and adjust your plan as needed to help you keep reaching your health goals.
Our team works across five specific Aged Care Planning Regions: Caboolture, Darling Downs, Logan River Valley, South Coast and Sunshine Coast. Wherever you’re located across these regions, you can access dependable aged care services, allied health and local services delivered directly to your home, where you feel most comfortable.
CHSP eligibility is based on your age and support needs. You’ll generally need to be 65 or older, or 50 or older if you’re an Aboriginal or Torres Strait Islander person, and require a little support to continue living safely at home. An aged care assessment, arranged through My Aged Care, will confirm your eligibility criteria and the entry level support that’s right for you.
As part of the CHSP, most CHSP clients are asked to make a small client contribution toward the services they receive. If you think you might qualify for a co-payment waiver, let us know — we’re here to help.
Yes, to access our CHSP services you’ll need a referral and an assigned CHSP code, which you can obtain through an aged care assessment with My Aged Care. Once approved, we’ll ask you to sign a simple service agreement before starting your sessions. If you don’t yet have a referral, visit the My Aged Care website for more information and to check your eligibility.
CHSP provides entry level support for older Australians who need a little help to stay independent at home. If your needs become more complex, you may be assessed for a Home Care Package, which offers a higher, more coordinated level of home care funding. Your aged care assessment will help determine which program best suits your individual circumstances.
Once we’ve received your referral, one of our friendly team members will contact you to confirm your details and discuss your goals. During the first appointment, we’ll conduct a comprehensive assessment in your home, covering your mobility, daily tasks and safety. From there, we’ll work with you to create a personalised care plan designed to enhance your independence and quality of life.
No, Talk Health Therapy exclusively provides in-home services for CHSP participants. Our allied health team comes to you to provide personalised care in the comfort and familiarity of your home, no matter where you’re located across our Brisbane and South East Queensland service area.
Yes, we will arrange to go to different venues as needed. However, as many venues charge a fee, we’ll discuss this option with you before your consultation. Please note that your first consultation is always conducted at home, and ongoing therapeutic intervention will be provided as discussed.
Yes, we organise group classes for land-based therapeutic exercise and group hydrotherapy. These are run by one of our physiotherapists or allied health assistants to help you maintain your strength and improve your fitness.
