The Commonwealth Home Support Programme (CHSP) is a government-funded initiative designed to help older Australians become more independent, more active, and safer in their own homes.
Through the CHSP program, our team of qualified Physiotherapists, Occupational Therapists, Speech Pathologists, Dietitians and Exercise Physiologists provide personalised in-home support designed to meet your unique health and comfort needs. We are dedicated to helping you maintain your independence and enhance your quality of life at home.
With the help of our mobile Physiotherapists, you can stay active and improve your physical strength with our personalised at-home exercise programs, mobility, and fall prevention strategies.
Our team of Occupational Therapists collaborate closely with you to recommend suitable assistive technology, suggest simple home modifications, and conduct assessments, all aimed at helping you maintain your independence and enhance your safety at home.
Our team of qualified Speech Pathologists is here to assist you in managing any challenges with speaking or swallowing, improving your communication skills, and promoting safer eating habits.
Our qualified Dietitians work with you to offer expert nutritional advice and create personalised meal plans, supporting your overall health and boosting your daily energy levels.
Our team of exercise physiologists is dedicated to supporting older Australians in improving mobility, managing chronic conditions, and achieving better health and well-being.
We start with a detailed assessment to understand your unique needs and goals, and how these fall under the CHSP. This includes evaluating your mobility, daily living abilities, and your overall state of health, as well as learning what’s important to you to best align our services with your lifestyle and health priorities.
After the assessment, we’ll collaborate with you and your carers to outline achievable goals to help you maintain independence and enjoy daily life. We create a tailored care plan that focuses on your specific challenges, whether it’s improving mobility, making your home safer, or helping support your nutrition or communication abilities.
With your overall care plan in place, our team then is ready to provide regular therapy sessions and services in the comfort of your home. We provide ongoing guidance, monitor your progress, and adjust the plan as required to keep you on track and achieve your health goals.
As part of the Commonwealth Home Support Programme (CHSP), most clients are asked to make a small co-payment for the services they receive. If you think you might qualify for a co-payment waiver, let us know—we’re here to help.

Yes, to access CHSP services with Talk Health Therapy, you’ll need a referral and an assigned CHSP code, which can be obtained through an assessment with My Aged Care.
If you already have a CHSP code, you can contact our team directly to start the process: simply reach out via phone at (07) 2111 6983 or email at info@talkhealththerapy.com.au, and we’ll guide you through the next steps.
If you do not have a CHSP referral, you can go to the MyAgedCare website for more information and to assess your eligibility.
Once we’ve received your referral, one of our friendly team members will contact you to confirm your details and discuss your goals. During the first appointment, we’ll conduct a comprehensive assessment in your home, covering your mobility, daily living tasks, and safety.
From there, we’ll work with you to create a personalised care plan that’s designed to enhance your independence and quality of life.
No, Talk Health Therapy exclusively provides in-home services for CHSP participants. Our allied health team comes to you to provide personalised care in the comfort and familiarity of your home, no matter where you’re located across Brisbane.
Yes, we will arrange to go to different venues as needed. However, as many venues often charge a fee, we will discuss this option with you prior to your consultation.
Please note as well that our first consultation is always conducted at your home, and then ongoing therapeutic intervention will be provided as discussed.
Yes, we will organise group classes for land-based therapeutic exercises and group hydrotherapy exercises. This will be completed by one of our physiotherapists or allied health assistants to help you maintain your strength and improve your fitness.
